That is exactly what's happening. It reappears when LO is closed.

MacOS 10.14.3; QS v. 1.5.6. QS Crashes a every week at least once. I have not been able to figure out why it's happening. I do usually send crash
reports.

OK, that explains a lot. That version had a bug in the update system that caused a crash. So once a week, when it checks for updates…

The current version is 1.6.0. Download it manually from https://qsapp.com/ and give that a try.

QS does not appear in any space until LO is closed.

Well, I don’t know what to make of that. Could it be taking over the shortcut you use to activate QS?
