As usual, Dan is right. If you have udev working you will have a /dev/video0 
device as soon as you load the driver, and the cam is plugged in, of course. 
Recently, I was given a cheap logitech quickcam webcam. Kernel 2.6.18 has a 
basic support for it, but my older kernel hadn't. Well, I asked google and I 
saw several drivers that I thought had to work. But until I found the right 
one, nothing. I even tried to use makedev and cheat a bit, but don't do it. 
If it doesn't work, it doesn't. I'm saying this because you might think that 
it should work and you might be wrong, not the kernel. So what you have to do 
is make sure you find a working driver. You will know when it works. Then, 
you can try xawtv, camorama or even mplayer to test it, before using ekiga or 
any other software.
