I got cdparanoia and am pretty familiar with it.
If I'm really into "forcing" the issue, I'd just enable the
"Digital audio extraction" in XMMS:
Options > Preferences > CD Audio Player.

I was thinking more in terms of maybe having this "feature"
in the kernel (as we take it for granted with Analog audio)
and my (and some simple applications) not being aware of it.

I apologize for butting in on your 2.6.21.5 tribulations
(I got the whole story as a bonus of being temporarily
subscribed to the blfs list - on account of my digital question).
and further confusing you with a couple of my humble thoughts:
1. I did stumble on a bug in 2.6.21.one.
   Fixed by Alexander E. Patrakov for me on this list.
   I'm solid and stable on 2.6.21.3 now.
2. In the kernel update/recompile routines I always do
   mv .config config-2.6.21.x-y         # save
   mrproper                                # wipes out '.config'
   cp -p config-2.6.21.x-y .config
   make menuconfig                         # and never forget to
   make oldconfig
