Hi Cliff,
A while ago, "xmlcatalog" had an issue that generated bad XML catalogs. I
think it has been fixed in version 2.9.2, and certainly in version 2.9.3 of
libxml2.
I suggest that you install the latest version of libxml2 (2.9.3), that you
remove /etc/xml/{catalog,docbook}, and that you run again the configuration
instructions in docbook-xml and docbook-xsl.
