I think a lot of what you are struggling with in scanning has to do with
the OCR program you are using.
If you can change your scanning settings to recognize columns you may be
able to get rid of the problem of merging text from side bars into the
main text on the page.
I'm using Open Book from Freedom Scientific for my scanning.  I keep it
set to recognize columns all the time.  Often this works, but not always.

Pictures are often simply eliminated in the translation process, perhaps
leaving a few symbols in their place.
Picture captins will remain, since they are text which the sOCR program
will recognize.

Any tables or graphs with letters and numbers in them will be recognized
and reproduced, but not necessarily in a readable form.

There may be times you do not want to recognize columns.  For example, if
the page you are scanning has a table with columns you wil likely be
better off unchecking the "recognize columns" feature.

My program also has a feature that can do two page scanning.  This way
you can lay a book on the scanner and the result will be two pages
scanned and translated at the same time without confusion between them.
