where they have step-by-step PHP/HTML tutorials, or, the simplest version of an email contact form is to use a simple HTML form where the action is in fact mailto:[EMAIL PROTECTED] etc. etc., but you might do best to in fact get onto either the PHP mailing lists, or else blind-programming ones to get more details. FWIW, a PHP email form is also very simple (literally redid one around 2 days ago using around 15 lines of actual PHP script), so you can also contact me off list if you'd like an example thereof.
