Alt arrow down normally does the trick on difficult combo boxes. If this is not working it may be a case for the Jaws Cursor to have a go. Do you have a link to the offending page and combo box to see if we can get it to work?
Unfortunately some pages are simply not accessible to speech software but we can have a go.
