What operating system? If, for example, on XP, since you are actually running under an identity/user profile as such, you can in fact assign a password to it, and make it get entered when entering the active session, and you can also do something like make it activate the screenreader after a period of inactivity, and request the password when it's then reactivated.
Can't off-hand remember where all these settings are in fact activated, and I'm thinking of XP Pro (might also work on home), but I think the password changine happens under the user account settings under control panel, and the screensaver change is either on the display settings, or somewhere to do with standby mode or something.
