HI Alana. Apple has certainly make great strides in their efforts to provide accessibility. I'm impressed with the willingness they have and with the progress they've made. I don't' use the Apple screen reader myself, but there is a 2 or 3 part series about it on the blind cool tech site. www.blindcooltech.com
This demonstration will give you a pretty good idea of how things are done using the Apple screen reader, Voiceover. Check it out. I think your questions will be more than answered by it. A few blind users use the Mac daily and even exclusively. The alex voice is very good on it, too. Hope this helps.
