yes, you need to have this check-box, unchecked.
also, you may want to enter on, apply to all folders button.

Could it be that "Hide extension for known file types" is checked?
- Start windows explorer ( windows key + e )
- On the menu bar go to Tools, and then Oo Folder Options.
- Move to the View Tab by pressing Ctrl Tab until you her View.
- Tab down to the list/tree view and scroll down until you find "Hide
extension for known file types" and make sure this check box is not
checked. If it is checked remove the it by pressing the space bar.
- Press Alt+l to apply to all folders and then Tab to Ok and press space
bar.
JAWS should now read file extensions.
