I think you need to help us a bit by being clearer .  
What exactly are you finding difficult ? 
Have you listened to any of the many audio Help files built into Text Aloud
?
It would be better for you to listen to these first than for us to start
reproducing all the instructions they contain on this list.
In any case the program is pretty intuitive and simple to use.
To get started load a text file using the file menu.
Then go to the speak menu and select speak current article to file.
It is probably best though to work through the audio help menu  as well.
Post another question if you do not understand the audio help .  Make sure
you are specific about what bit you do not understand and we can probably
help.
