I presume that you have tried a reboot?

Not necessarily a solution, but a possible workaround, is to hit alt plus
space then C to close the window.

I find this tends to work if there is an alt F4 problem. The fact that you
can alt tab seems to indicate that the alt key is actually being recognised.

Subject: [Blind-Computing] Alt Key Intermittently stopps working

Hello all. My friend was navigating a site using IE 8 and Jaws 10 and could
not Alt f4 out of the site when she was finished. In fact everytime she hit
the alt key Jaws would speak the word "alt" but the alt key itself failed to
work.
She tried both alt keys and still had no success. Each time she presses alt
she just hears a ding sound but nothing happens.
She was able to alt tab away from the site and go to her e-mail client and
the alt key worked just fine while there.
Any suggestions as to how to fix this alt key would be greatly appreciated.
