Probably you have checked the mixer checkBox in windows volume control.
I am sorry i can not give a detailed solution as i am not using vista,
but i guess that you have the stereo mixer checked, that's why people
can not hear you as your sound card exports only the sound that it
produces from inside and not the sound that comes through the
microphone. I hope someone else with vista would help you more precisely
