There is a certain shareware called Softany WinCHM which should do the
job well. Even with a screen reader, the tree view is presented as it
will be after compilation. It just asks where your HTML files are
located and thereafter organises a default Table of Contents and topics
based on their titles (those between the <title> and </title>). You can,
however, do any other editing after that.
