1. Insert+n for NVDA menu, then arrow down to preferences and enter
2. Arrow up to speech dictionaries and enter
3. Choose the dictionary you want - the default one is generally for all 
voices, the voice one for the current one you use and the temporary one is 
just provisional, well, temporary :) press enter to open it
4. There is the list of entries, or if you still haven't added anything 
there is nothing yet :) Press tab to add an entry and space to confirm that
5. There are some fields, the most important will be the pattern and 
replacement ones so put the word or strings of symbols you want to fix into 
the pattern field and whatever you want it to be replaced with into the 
replacement field
6. By tabbing further you'll find checkboxes where you can specify if you 
want the entry to be case-sensitive and if it should have regular expression 
but that part needs some more advanced knowledge; you don't need regular 
expression for simple replacements so you can just leave it as it is
7. Tab to OK and confirm it by space, that will still get you to the list, 
so tab again to press OK there and you're done
