This really depends on what level of security the document creater uses.
Typically, I get a lot of documents that are images, with no text at all.
Sometimes Adobe 9 can pull some text out of them, but I've had far better
results by using Open Book's Freedom Import Printer to do OCR on the image.
Depending on how the document is formatted, you may have to play with the
decolumnization feature in Open Book to get the results you want.

If you are running JAWS and the file is saved in a non-graphic format, I
believe you should be able to read it by opening it in Adobe Acrobat reader.
I don't know anything about other screen readers.
