I know about Express Talk (http://www.nch.com.au/talk/index.html) or SJPhone 
(http://www.sjlabs.com/sjp.html). With the latter make sure in the options 
that using skins is unchecked to make it more accessible; oh, it has buttons 
you tab through to get to various items, not regular menus; otherwise it 
seems to be fine.
