I think this might be what you need:
With Visual Studio 2017 open, go to the tools menu, then choose Get tools and features.
This will open the Visual Studio installer on the workloads tab.
Tab down to the Game development with Unity package checkbox and check it, then finish the steps for installing it.

I don't use Unity myself, so can't comment on its accessibility.

Does intellisense / auto complete in Visual Studio 2017 work for you?
Mine stopped working after an update and I haven't found a fix for it yet.
I'm using NVDA 2017.4, Visual Studio 2017 version 15.5.7, and Windows 7.
I'd be interested to hear about your setup if it is working for you.
