I just looked into this. I’m guessing that you installed the “Metal” version of 
Retroarch? That was my first instinct as well, and sure enough I ran into the 
same issue as you, even after editing the configuration manually. I just 
reported this to the retroarch developers and you can follow the progress along

The good news is that, like I said in the issue description, the regular 
non-Metal version works fine so you can install that one to get speech. To do 
this without sight, in finder, press Command-Shift-G to bring up the go to 
folder dialog, and type in ~/Library/Application Support/RetroArch/config
In there you should find a file called retroarch.cfg, which you’ll want to 
open. It’s better to use a plain text editor like Textmate to do this, but if 
you want to do it with text edit make sure to go into the edit 
menu>substitutions and turn off smart quotes. In that file, the very first line 
should be accessibility_enabled = "false", just change the "false" to "true" 
and you should have speech. If you're not comfortable doing this yourself, you 
can also just ask your husband to look for the accessibility option in the 
menus again which should show up in this version.
