It takes a little practice.  After scoring it, then bump on the inside 
of the pattern but don't knock it all of the way out.  This breaks the 
chunk out but doesn't tear the paper on the other side.  Then go around 
it a second time with your knife to cut the paper on the other side.  

If you try and bump the whole thing out in one step, the paper on the 
far side holds it, causing the break to be uneven.  It helps to have a 
really sharp knife too to keep the paper from tearing instead of 
cutting, especially on the far side.
