I have also saw the single phone jack and single cable jack on one plate.  
Sounds like more than that is getting crowded.  Back in 1980 we were tasked 
with adding a telephone jack and cable jack into each student room in this one 
dorm. I looked for the two on one face plate.   No one was making such a animal 
back then so I had to use a cable face plate and attach a surface phone jack on 
the side of a metal Wiremold box.  Could of made a fortune, but oh well.

  At Home Despot or Lowes, I found face plates with multiple blanks in them. 
  You then purchased blocks to insert into each blank. They had phone 
  blocks, cable blocks, and a couple of other various things. So you 
  basically assemble whatever you need in the one face plate.
