#449: Expand sticky header to work across all screens

#431 modified the sticky header to use the breadcrumb bar or the full
header, depending on the screen size. This means that there is nothing in
particular stopping us from adding such headers to all pages.
This should be as simple as making the appropriate stickyscroll javascript
available to all templates and removing anything that loads the javascript
on specific pages.
