Hi Adrian,

To get around the problem in the Southern NSW we stopped using the plastipak 
syringes and changed to the BD precise which were already setup in most of our 
syringe drivers/PCA's etc.

If anyone has been following these they will be aware that earlier last year BD 
changed the design of this product, with the new version exhibiting somewhat 
higher friction that the original. This lead to syringe drivers having issues 
with false occlusion alarms. Safety alerts were published widely advising 
people to be aware of this and re-calibrate pumps or increase occlusion alarm 
settings as required.
This has particularly started to show up here in SA over the last few months, 
with most of the big hospitals having issues that has seen some of them needing 
to change their delivery protocols as the recal or changing alarm settings is 
not possible/appropriate in some cases.  It is particularly a problem with PCAs 
due to the stop/start nature of those deliveries.
Is anyone in other states having similar issues? The TGA do not have a lot of 
reports about it (maybe only ours)......but I have a suspicion that not many 
people are reporting into the TGA all that much anymore.
