On BoltWire, I've turned off basically all the plugins until I can figure out what is causing these intermittent server configuration errors. It's possible these is a bug either in some plugin, or even the core script. But I haven't been able to track it down yet.
Once I get it figured out (if?), I'll turn everything back on. Sorry I haven't had more time to really work on this... If it is not working on a fresh installation, there must be some additional problem. Not sure. I'll look into it. Ahhh... I see what it is. If you look in code.embed.gui you will see a $$icon on each line. These need to be changed to $$img. I'll release an upgrade to the backup file here momentarily. If you haven't done it yet, you will need to rename your farm/icon folder to farm/img also.
