I tested a clean install--and it seemed to work fine... But looking at the code action.create should not have the [] in the button (--> name=submit[] ). Try deleting them and see if that doesn't fix it.
Also check to make sure you don't have a local action.create page. If you do, update the markup for the buttons and the if statement in your local version.
