Add monitoring support

There should be basic monitoring information available for the hubs, e.g.,
#subscribers, their backlog, #topics, CPU
load, publish rate etc. JMX integration to be explored.
Similarly we should have this for BookKeeper. Each should probably be done as a
subtask of this JIRA.
