It would be good to get a more detailed description of both the btree &
skiplist disk formats.
For btree, I'm not sure the approach makes sense. The workload is append only,
so with a tree, you're going to be constantly skewing to the right, and having
to rebalance.
For skiplist, how many levels of skiplists do you envisage? The index file
format you describe is effectively the top level.
