I have not got a solution for this yet, but will look into it today.

The Boost Files section contains an update that fixes both of these :). Let me know if there are any other features or display problems and I'll try and fix those.
NOTE: This version of the boostbook.css does not contain the changes you made for consistency with screendisplay? and programlisting.
NOTE: I haven't had chance to work on the syntax highlighting yet, but intend to do this hopefully sometime next week.
