I've done the changes you suggested and everything goes right. The document type I defined was not library, so I had no TOC. The css change (borrowed from Quickbook itself) and the jam parameter for boost root were fine.

That's the question I always wanted to ask the BoostBook guys. Why don't we have a TOC when the document is not a library?
