If a document is meant to be included as part of a bigger document, I
think it should not have a header. Maybe we should issue a warning

What if the included file is just meant to supply the header?
OK, so the header should neither be ignored nor disallowed in included
files. But it should be an error to specify the header twice or to put
the header anywhere except the top. I'll take another crack at this.
