I'm guessing that <classname>, <functioname>, etc. only produce a link
if doxygen has been run on the source code.  Shouldn't that be
mentioned in the above together.html.  Also, I missed reading
about these elements the first several times I looked at the
boostbook documentation because the titles:

  Bringing Together a BoostBook Document
  Linking in BoostBook

weren't specific enough to catch my attention.  IOW, I already
knew about docbook's <ulink> and <xref>; hence, when I glanced
at the first paragraph, it didn't prompt any further attention.

I suggest dividing together.html into a section dealing with
the docbook links, and another for the doxygen links.
