Hello,

it looks like Boostbook can't handle classes declared entirely in .cpp files.
While doxygen produces information about them, the boostbook file created from 
doxygen output does not mention such classes.

Is this something fixable? For Boost, it's not very important but generally 
it's good to be able to document implementation classes that have no headers.
