Fixed. As for Qbk syntax for strike-though, I seem to agree.
We very seldom need it anyway.
As per this discussion, I have removed the simple-strikethrough syntax
from QuickBook. Before, you could get strikethrough font with -this-
syntax, but it was too error prone. QuickBook still supports [-this]
syntax for strikethrough.
Also, I have changed QuickBook's docs to use the "article" DocBook type
instead of "reference". That way, we get a Table of Contents on the
first page. Huzzah.
