Historical. QuickDoc didn't have the simple formatting. When
it came to supporting that, I tried to follow common conventions.
But then, the old [^teletype] was retained for backward
compatibility. Perhaps we can fix that with 1.3 plus a version
switch?
You don't say it... But I gather you mean adding [=teletype] to the
existing syntax?
