I've had some success with qbk-mode. I've got most of the basics
working and have finally eliminated all of the pathological
backtracking that happens when quickbook's own documentation is
fontified. Tables cause no end of grief because, e.g.,

              [*this isn't bold]

can be bold or not depending on the table context, but I have some
ideas for resolving that.

I can't check it into Boost yet because it contains some GPL code
stolen from rst-mode to get me going. We'll have to rewrite that
stuff before I can make it Boost-licensable. In the meantime, if you
want to look at it,

https://boost-consulting.com/svn/main/boost/1.33.1/backports/boost_1_33_1/tools/quickbook/qbk-mode.el

And if you want to help by hacking on it (please!) let me know.
