I have test it and I can not reproduce it here. What OS are you using?
John have reported and I was able to reproduce it that the refinements
labels do not work sometimes. Since you have try to use cse once, if
you want to help me getting it working it will be great. Just download
the docs and look for:

<div class="heading_search_box">
   <form id="cref" action="http://google.com/cse";>
       <input type="hidden" name="cref" value="" />
       <div> Search Boost </div>
       <div>
       <input class="search_box" type="text" name="q" id="q" size="40"
maxlength="255" alt="Search Text" />
       </div>
   </form>
</div>

You have to add an uri in the value attribute of the "cref" input element.
I have been using this one

Do not feel force to do it, just an idea :)
