Yes, you would think that installing the firmware fudge to the
BrailleNote would just be a matter of asking RFB and D, but I
guess they want to ensure their digital books don't get into the
hands of a non-eligible blind person, e.g. a heathen foreigner!

So they may let Humanware install a DAISY reader that can have
the RFB and D mod installed, but only provide the CD with the mod
program on it to eligible American members of RFB and D.

With a Victor Reader or Plextalk, you have to either buy it
directly from RFB and D, or physically send it to them for the
modification to be installed by them which I think is $10 plus
return shipping by insured UPS.

I think with DAISY pc software, they will mail you the
modification on cd and require you to send it back to them after
you are done with it.
