Yes, it is a rare occurence. It is usually caused by malfunctioning keyboard spring (there are two notches under the key that keeps the keys in place). Once a while ago, I had a faulty keyboard where my ENTER key (on the BT keyboard, of course) on my old Classic would fall off. I've checked out underneath the keyboard and found that one of the notches was broken. I've put the key back in its place and it worked fine. Once, I've even took out some keys from my laptop and put those keys back on their correct spots (and the amazing thing is that I've done it 100% accurately). There is a little spring that controls key action. Even though a key is missing, you can use this spring to type. This type of keyboard malfunction usually takes place on the right side of the keyboard (on the BT, it is usually dots 6 or 8; on the QT, all keys to the right of the l key are usually targeted).
