When I sent my mPower in (thinking it would just be for a battery change,
but deciding to do the trade-in for the Apex), Humanware sent UPS to my
house, and everything was all set, and the UPS guy just stuck the paper to
the box, and I was good to go. I didn't have to fill out anything.
