Diane, I have a separate folder on my BN that is called "forms." There are probably 10 or so documents in that folder, each of which is a form template I've written. When I write notes on a client I use a blank document titled with the client's name and use the insert command from the block commands menu. Do a b with space, then i for insert, then the folder name which in my case is "forms" then the document name which is the name of your form. It might be something like "intake." The information from your form will immediately be inserted into your new document. If this doesn't make sense it's because I'm at the end of a long day which began with a big children's recital for which I was responsible.
