I customized my BN too. I have from left to right, advanced, back, next then 
previous. When I'm reading a document on the BN, the advanced and back move 
line by line and those are the only keys I mess with while reading with the 
display, however when the Braille display is being used with Jaws and the 
computer, the previous thumb key moves the display forward by sentence and 
the back thumb key moves it back by sentence, not sure what the other keys 
do yet.

The previous and next thumb keys can also be set to read up and down line by
line and up and down by paragraph.  I have mine set to move by paragraph.

I don't know what screenreader you are using so I can't comment completely
about the thumb keys.  I use WindowEyes.  The BN's thumb keys are numbered
from left to right, thumb 1, thumb 2, thumb 3, thumb 4.  I wish Humanware
would label the thumb keys that way, too.

I didn't like the way the thumb keys on the BN were set, so I used the
custom option to change them.  I prefer next and back to be on the ends and
previous and next to be in the middle.  So, the WindowEyes BN hot keys
worked great for me as far as moving display lengths went.  But I changed
the two middle keys, pressed at the same time,   would read forward
paragraph by paragraph and the two keys on the ends pressed together would
read back a paragraph at a time.

The more keys and buttons a braille display has, the more hot keys you can
use with WE.  Even more keys would be available if all of the keys on the
QWERTY keyboard could be used as hot keys.  I believe the manufacturer of
the BN, or whatever braille display is being used, creates the hot keys
which work with the screenreader.  So Humanware created the WE hot keys.
