One of the main causes of poor Crema is stale coffee. Fresh coffee straight from the grinder may be all you need. However, a sacrifice to the Kona God is always in order. Crema may not be all it's cracked up to be. See: http://www.jimseven.com/2009/07/06/video-1-crema/
