Your hypothesis that the flow of water from the three holes in the group head 
is not even is feasible. But I would be inclined to focus on whether the face 
of the filter basket (nicely exposed by the Naked PF is parallel to the ground. 
 (You did mention that you could shift the pour back to the center by lifting 
the front of the machine.)  Then remove the Screen on the group head and be 
sure it is completely clean. Screens do become blocked in places.

 

My next suspect is distribution within the basket.  If you grind into the 
basket while it is mounted in the PF (typical of shop practices), Try removing 
the basket and filling it, stirring the grounds with a large needle such as 
those used in leather or sail makers shops,  leveling it and finally tamping it 
before placing it in the PF.  If the pull still comes out by the handle, then I 
would consider the blockage in the group head and a next target.

 

In my experience, the majority of pour problems are the result of flawed 
technique, not machinery.

I am using a naked portafilter.

I took a good look at the flow and noticed that the stream was not coming out 
of center of portafilter. I was coming from a point close to the handle.

I noticed this recently and had no idea when the issue started.

 

I raised the front edge of the Brewtus II by about 1" and now the stream is 
coming from the center. 

This sort of fixed the symptoms.

But, what was causing the problem? Is it something inside the brew head? I had 
taken that apart about 6 months ago and replaced all the internal parts.
