Put a toilet latch on it.  The ones I have each have an arm that goes
out over the toilet lid and prevents it from being opened; when you need
to open it, push the arm back and lift the lid (with or without seat) at
the same time.  When you're done using the toilet, put the lid back down
and the arm swings back into place.

As for the later question, you start out with a potty.  :)
