It's the ammonia (= waste product), which helps against the sting/bite and itching, not sure whether it's also an antiseptic (though pee is sterile except for bladder/ urinary tract infections). They also sell bite relief sticks with ammonia in them.
And they sting like the dickens the instant you apply the stuff, but an hour later, if someone had an untreated bite for you to compare yours to, BOY are you glad you put on the stuff. :) (Mom bought it for mosquito bites, which we always had lots of until we started consuming a lot more garlic.)
