I don't know if this is true for all Alva users but after version 6.1 of BRLTTY I found it necessary to physically start BRLTTY and then restart NVDA to get the braille going correctly.  Thinking that my display is some 30 years old I put this down to age and got on with it!

Now with the release of 6.4-1 all is working correctly again ...

I'm using an Alva via usb/serial on windows 10 with NVDA 2021.3.1 and now BRLTTY 6.4-1.

Hope this may help others and the developers.

Regards

Martin Murdoch.
