Mine is doing it with the machine sitting beside the computer (ok about 8 inches away and a bit higher) and with my one hand only reaching to double check the display. I may need to some experimenting on a windows machine and be sure the device works on something I have full install instructions for.

I've managed to reproduce this error with a properly paired device here. I'm
able to do so when the device and the host have poor connectivity (too far
apart, my hand is covering the device's antenna, device has a low battery,
etc). In this circumstance it seems (I'm guessing a bit) that the device and/or
host doesn't disconnect properly. Turning the device off/on, and then ensuring
that it's connectivity with the host will be good, seems to resolve the problem
(at least in this case).
