I have not tested BRLTTY with Braille Star 40 on USB for a long time.
However, I had the same problem when running the device on bluetooth. I
solved that situation by putting the following two lines into
/etc/rc.d/rc.local file:

cd `readlink -f /sys/class/bluetooth/hci0`/../../../power
echo 20 > autosuspend
