- cost (time and/or money) of proctoring (setup and monitoring) a
hands-on, interactive testing. (The costs may also involve actual hardware
too.)

I do not understand how setup and monitoring can be more expensive. Which setup ? Just launch a CD. And for the monitoring, this is exactly the same thing as for usual tests.

- not enough time to cover all the needed topics/skills.

This is why everybody now tend toward adaptive testing. You do not have 80 questions anymore, but only 30. For the same time.

How much time does it take to complete other professional hands-on,
interactive testing? Please share your experiences.

For non interactive testing, I can give what I know :

MCSE (Microsoft Certified System Engineer) : you have to pass 7 exams, each one consist of 50 to 85 questions, during about 2 hours.
Total = 455 questions / 14 hours / 840 euros

MCDBA (Microsoft Certified Database Administrator) : 4 exams of 55-70 questions, 2 hours each.
Total = 250 questions / 8 hours / 480 euros

OCA (Oracle Certified Associate) : Very basic certification. I do not remember well, but 2 exams of about 60 questions in 2 hours each.
Total = 120 questions / 4 hours / 240 euros

I heard about IBM certifications around 10 to 15 hours too.
