The survey was created using a piece of open-source automated survey
software, which is apparently crap.  What I meant to say (in the email
to which I believe you are referring here) is that the group has an
entire community to draw on if they need better software than what is
currently available.  I'm not volunteering - I'm already way behind on
the new site for the FreeBSD Foundation...

If the group has any kind of budget - and if it does not, it could
perhaps apply to the FreeBSD Foundation for support - there are
companies out there that make a living from hosting surveys for a
reasonable amount of money.  For a slightly larger amount, they will
gladly help you design them and analyse the results.  One of them is
QuestBack (http://www.questback.com/).

(I have no connection with QuestBack other than having previously
worked for a company that used their services for both internal and
external surveys)
